Thames Reach Charity is seeking a Lead Worker to join the Hounslow Reach Team. You will oversee floating support services, assess referrals, and provide resettlement advice to sustain tenancies and support client independence.
You will collaborate with housing providers, referral agencies and health services to deliver flexible, person-centred support, co-produce plans with clients, and lead a team of staff, volunteers and trainees.
