Key Responsibilities
- Provide specialist assessment, interpretation and management of complex physical, emotional and practical care needs for patients and their families.
- Take overall responsibility for a caseload of patients, accepting referrals and working unsupervised.
- Collaborate with the multidisciplinary team to plan, review and improve patient care, ensuring services are evidence‑based and cost‑effective.
- Lead and support the delivery of high quality palliative and end‑of‑life care, integrating education and training for staff.
