Permanent part‑time role (approximately 20 hours per week) at Chesterfield Royal Hospital NHS Foundation Trust. Location: Chesterfield, Derbyshire, S44 5BL. Salary: £38,682 to £46,580 a year.

Key Responsibilities

  • Provide specialist assessment, interpretation and management of complex physical, emotional and practical care needs for patients and their families.
  • Take overall responsibility for a caseload of patients, accepting referrals and working unsupervised.
  • Collaborate with the multidisciplinary team to plan, review and improve patient care, ensuring services are evidence‑based and cost‑effective.
  • Lead and support the delivery of high quality palliative and end‑of‑life care, integrating education and training for staff.
