An exciting and rewarding opportunity has arisen for a Band 5 Community Learning Disability Nurse working within an established Integrated Team alongside an MDT which includes Social Workers, Community Learning Disability Nurses, Psychiatry, Psychology, Physiotherapy, Occupational Therapy, Speech and Language Therapy, and Clinical Nurse Specialists. Durham County Council are the lead authority of the Integrated Learning Disability Team.

Candidates will use the Durham County Council system Azeus as their primary recording system with access to TEWV recording systems and NHS mail. The team works collectively and collaboratively with the wider MDT to ensure a high standard of care is provided and ensure people with a diagnosed learning disability and their families and carers are supported by delivering high-quality clinical care utilising the Care Act and TEWV clinical pathways, commissioning packages of care to meet assessed needs and signposting to community services. The service operates Monday to Friday.

Main duties of the job:
  • The successful candidate will have a genuine desire to improve the mental health and well-being of adults with learning disabilities and behaviours that challenge.
  • They will work as part of a MDT delivering high-quality secondary care and enhancing community services, working with individuals who require specialist interventions using trauma-informed care and a recovery-based approach.
  • Must be motivated, flexible, and able to communicate effectively, and will be expected to travel independently and have access to a suitable vehicle for business purposes.
  • Management of a varying caseload involving nursing assessments and interventions.
  • Delivering treatment and interventions, monitoring and evaluating outcomes.
  • Care Act assessments and reviews.
  • Signposting to community services.
  • Commissioning and review of care packages.
  • Reducing restrictive practices.
  • Multi-disciplinary working to ensure better outcomes for the individuals we work with.
  • Maintain good communication, documentation, providing reports and information within agreed timescales.
  • To adhere to legal frameworks/policies and procedures e.g. Care Act Safeguarding, Mental Capacity Act, and Mental Health Act.
  • Maintain their NMC Registration/PIN ensuring compliance with the NMC Code/revalidation.

Job responsibilities:

  • The post holder will deliver a quality service and ensure the individual is at the centre of their care ensuring they are fully involved in all aspects of assessment, planning, and care and treatment as appropriate, including being professionally and legally accountable.
  • The service is looking for RNMH or RNLD with relevant LD experience or who have a willingness to develop; staff must be able to care coordinate and should be able to work well independently as well as part of a wider team.
  • Will be responsible for managing a caseload under the supervision of a senior nurse/management, providing assessment, advice, and support to clients, carers, and other professionals.
Experience:
  • Current professional registration with Nursing & Midwifery Council RNMH or RNLD (RNSC - CAMHS only) - RGN (If appropriate).
  • To be recognised practice assessor/mentor or work towards in an agreed timescale.
  • Trained Clinical Supervisor (within 6 month probationary period).
  • Evidence of Continuing Professional Development.
  • Working with people with mental ill health/learning disabilities.
  • Working in a person-centred therapeutic organisation.
  • Working collaboratively with service users and their families/carers.
  • Working in a multi-disciplinary team.
  • Working in partnership with other agencies.
  • Evidence-based knowledge of best practice in caring for patients in the designated field.
  • Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act).
  • Understanding of Safeguarding and its application in practice.
  • Clinical Risk Assessment and Management and its application in practice.
  • Clinical Governance and its application in practice.
  • Confident working knowledge of clinical medicines management, including administration of injections.
Knowledge and skills:
  • Communicate complex and sensitive information effectively to patients, carers/families, and all members of the multidisciplinary team.
  • Work effectively as part of a multidisciplinary team.
  • Undertake lead professional/care co-ordinator responsibilities (following probationary period).
  • Manage own time and prioritise effectively.
  • Co-ordinate and prioritise the activities of the ward/community team.
  • Motivate and work positively and assertively with other members of the team.
  • Provide (following probationary period) effective teaching, training, and clinical supervision.
  • Use approved techniques in physical intervention (following training).
  • Demonstrates recovery-focused practice.
