This is an exciting opportunity for a part time substantive Qualified Nursing Associate to join the York and Selby Memory service. This is a newly created opening within our team that will support us to reduce the wait time for patients and their carers in accessing our high-quality memory assessment and diagnostic service. The role will involve medication monitoring, physical health checks, group work and building links with other services.
We are a small, enthusiastic and supportive team who consistently get excellent feedback from both patients and carers.
Main duties of the job:The successful candidate will support patients who have recently been prescribed medication following a diagnosis of dementia. They will run medication monitoring clinics under the supervision of a senior practitioner and be responsible for delivering physical health checks as needed. This service will also be offered in the patient's environment, for those who are unable to attend clinics.
In addition, there will be opportunity to run Cognitive Stimulation therapy groups, build upon links with other services that support people with a diagnosis of dementia, to ensure patients and carers have access to good quality support following discharge from our service. They will be a valued member of the team, working closely with senior practitioners and medics, with opportunity to attend the weekly multi-disciplinary team meeting. There will also be an expectation of engaging in supervision as outlined by the trust. It is an exciting time to join the service as we work together to find new ways of working to reduce our waiting list and further improve the experience of those that are referred to us.
Applicants must already have completed the Nursing Associate foundation degree and be a registered Nurse Associate on the NMC register.
Person Specification:- Qualifications: Registered Nursing Associate on the NMC register; GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent; Qualification in assessment, supervision and teaching in clinical practice.
- Experience: Up to date understanding of the scope of and role of the Nursing Associate in context of the nursing and multidisciplinary team and the organisation, and how the role may contribute to service development; understands and acts in line with NMC professional standards for practice contained within The Code; knowledge of when to seek advice and elevate to the appropriate professional for expert help and advice; understand requirements for NMC professional Revalidation; understanding of the importance of following procedures and treatment plans; experience in a health care setting.
- Knowledge: Experience of working in teams under appropriate supervision as part of a multidisciplinary team; insight into how to evaluate own strengths and development needs, seeking advice where appropriate; experience of providing and receiving complex, sensitive information.
- Skills: Ability to take part in reflective practice and clinical supervision activities; ability to organise and prioritise own delegated workload; ability to deal with non-routine and unpredictable nature of workload and individual patient contact; ability to communicate effectively (written, verbal and non-verbal communication) with patients/relatives and carers and all members of the multi-disciplinary team; ability to develop effective and appropriate relationships with people, their families, carers and colleagues; ability to support, supervise and act as a role model to nursing associate students, other learners and health care support workers as required within the clinical setting; must be able to demonstrate the English language proficiency level required for this post.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
Salary: £27,485 to £30,162 a year pro rata, per annum.
