Location: Roseberry Park Hospital, Middlesbrough (primary office), with a dedicated hot‑desk office at James Cook University Hospital.
Service: Urgent Care Services – provides Mental Health and Social Care (MHSOP) and Adult Mental Health Liaison across Teesside, operating 24/7 to meet national targets of 1‑hour ED referral and 24‑hour ward referral.
Main duties of the job:
- Provide specialist clinical input in older people’s mental health within the acute hospital liaison team.
- Review patients previously assessed by ward‑based teams, performing assessment, treatment and discharge planning for those with comorbid mental health problems.
- Offer specialised advice on capacity and risk issues and support acute teams in recognising and managing mental health needs.
- Coordinate with local community and inpatient mental health services to ensure appropriate follow‑up and support post‑acute care.
- Take part in Mental Health Act assessments and, where required, act as responsible clinician for detained patients.
- Collaborate with the Liaison Operational Service Manager and clinical leads to plan and develop services in line with the Trust Integrated Business Plan.
- Advise the acute trust on delivering the Treat as One agenda to ensure parity for patients with physical and mental health conditions.
