Exciting and rewarding opportunities have arisen for Band 6 positions within the recently formed NYY&S Intensive Support Service for people with a learning disability. The post holders will work as part of the multi-disciplinary Community Learning Disability Teams, addressing and supporting the health, psychological, emotional and behaviour needs of adults with a learning disability who have complex physical and emotional health needs.

The base will be predominantly in York at Systems House and will cover the North Yorkshire area including York & Selby, Northallerton & Richmond, Scarborough, Whitby & Ryedale, Harrogate/Knaresborough. The focus of the team is to provide an alternative to in-patient care in line with the transforming care agenda, focusing on people with a learning disability who are experiencing mental health difficulties, behavioural challenges, placement breakdown and medication titration, who should be treated in the least restrictive environment with the minimum of disruption to their lives.

The team may also provide some in-reach into AMH/ACUTE/LD hospital settings as liaison support when clients need to access inpatient settings. The IST operates from 8am-8pm on a 12hr shift pattern, 7 days a week. Candidates must have 18 months post-qualifying experience including sign off of appropriate preceptorship or competency framework.

Main duties of the job:

The successful candidates will need to have a genuine desire to improve the mental health and well-being of adults with learning disabilities and behaviours that challenge. They will work as part of an MDT delivering high-quality secondary care and enhancing community services, working with individuals who require specialist interventions using trauma-informed care and a recovery-based approach.

The successful candidate must be motivated, flexible, able to communicate effectively, and will be expected to travel independently to various locations within the NYY&S locality on a regular basis. Due to the nature of the role, it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010. A trust car lease scheme is also available.

The intensive support team is a newly developed team and the Band 6 clinical leads will be pivotal in developing and leading the team.

Detailed job description and main responsibilities:

The post holder will deliver a quality service and ensure the individual is at the centre of their care, ensuring they are fully involved in all aspects of assessment, planning and care and treatment as appropriate, including being professionally and legally accountable. The service is looking for RNMH or RNLD with relevant LD experience or who have a willingness to develop. Staff must be able to care coordinate and should be able to work well independently as well as part of a wider team.

The team will cover a 7-day service, enhanced hours working a 12hr shift pattern. The post holder will be responsible for managing a clinical caseload under the supervision of the Professional Lead, providing assessment, advice and support to clients, carers and other professionals, and delivering training. We are looking for people who are enthusiastic and willing to help support, develop and shape the team.

Job responsibilities:

The service is looking for motivated and forward-thinking nurses as part of the Intensive Support Team. You must be able to care coordinate and should be able to work well independently as well as part of a wider team.

The successful candidate will be responsible for managing a clinical caseload under the supervision of senior nursing colleagues, providing assessment, treatment, advice and support to clients, carers and other professionals.

Person Specification:
  • Current professional registration with Nursing & Midwifery Council - RNMH or RNLD
  • Evidence of Continuing Professional Development.
  • Recognised Practice Supervisor and/or Practice Assessor, clinical educator or equivalent. Must be achieved within 6 months
  • Trained in Clinical Supervision and providing clinical supervision OR willingness to work towards within 6 months
Experience:
  • 18 months post-qualifying experience including sign off of appropriate preceptorship or competency framework
  • Working collaboratively with service users and their families/carers.
  • Providing clinical supervision to individuals or groups or professionally appropriate.
  • Supervising or assessing students on practice placement.
  • Working in a multi-disciplinary team
  • Quality improvement activities
Knowledge:
  • Demonstrable knowledge to post-graduate level of evidence-based practice in caring for patients in the design
  • Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act)
  • Detailed understanding of Safeguarding and its application in practice
  • Clinical Risk Assessment and Management and its application in practice
  • Clinical Governance and its application in practice
  • Research and development methodology
  • Understanding of psychological models of care and treatment
  • Working knowledge of clinical medicines management, including administration of injections
