An exciting opportunity is available within the Living Well team at Terrence Higgins Trust. Funded by Fast Track Cities London, as part of a collaboration of HIV organisations, THT will be delivering an ambitious and exciting care coordination project aiming to support patients living with HIV who are struggling to engage with their HIV care and treatment.
The HIV Care Coordinator will be responsible for delivering a package of care and support to people living with HIV to identify and overcome barriers to engaging with care.
We are looking for someone who can successfully build collaborative partnerships with a number of different clinics and voluntary sector organisations as well as encourage and motivate patients to engage in their care.
The role is for 12 months (part-time, 3 days per week, based on site in HIV clinics in South West London).
