Position Summary
The Antibiotic Stewardship Clinical Pharmacist will serve as a unit-based pharmacist, verifying orders and providing specialized clinical services in antibiotic stewardship and infectious disease. This role will be a critical drug information resource for nurses, physicians, patients, and families.
Responsibilities
- Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals.
- Routine attendance on medical rounds (for decentralized shifts) with consistent documentation of pharmacist interventions.
- Deliver medication therapy management with a focus on antibiotic stewardship and infectious disease protocols.
- Ensure pharmaceutical care programs are appropriately integrated throughout the institution.
- Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to antimicrobial therapy management.
- Conduct Medication Use Evaluations (MUEs) with a focus on antibiotics/ID, and recommend necessary changes to pharmacotherapy practice.
- Serve as a resource in antibiotic stewardship initiatives, ensuring best practices and compliance with institutional standards.
Qualifications
Education
- Required: PharmD or BS in Pharmacy
Experience
- Required: Pharmacy practice residency or equivalent hospital experience (3 postgraduate years)
- Strongly Preferred: Infectious Disease training and/or experience in antibiotic stewardship
Certifications
- Required: Current Pharmacy license in Massachusetts
- Preferred: Board Certification in Pharmacotherapy (BCPS) or actively working toward certification
