We are currently recruiting for an experienced Adult Social Worker to join Hackney Council's Access & Duty Team.
Key Responsibilities:
- Undertake Care Act 2014 assessments and reviews.
- Complete strengths-based assessments and support planning.
- Manage a varied caseload of adults with complex needs.
- Carry out safeguarding duties including Section 42 enquiries.
- Work in accordance with the Mental Capacity Act (MCA) and DoLS.
- Develop outcome-focused support plans in partnership with service users and carers.
- Work collaboratively with health professionals and multi-agency partners.
- Maintain accurate and timely case records and reports.
Essential Requirements:
- Qualified Social Worker (Degree, DipSW or equivalent).
- Current Social Work England (SWE) registration.
- Recent Adult Services experience within a UK Local Authority.
- Experience completing Care Act assessments.
- Knowledge of safeguarding procedures, MCA and DoLS.
- Ability to manage an independent caseload.
- Strong assessment, report writing and communication skills.
Desirable:
- Access & Duty Team experience.
- Community Adult Services experience.
- Direct Payments and support planning experience.
- Experience working within a London Borough.
