JOB SUMMARY
We are seeking a dedicated and compassionate Reablement Assistant to join the Intermediate Care and Reablement Service. You will play a vital role in supporting adults to regain their independence, rebuild confidence, and maintain daily living skills helping them remain in their own homes and communities wherever possible.
JOB DESCRIPTION
- Support & Care Delivery: Assist service users with personal, social, and emotional needs in line with individual support plans. Encourage independence in daily living tasks such as cooking, cleaning, and managing health. Promote dignity, respect, and empowerment at all times, ensuring sensitivity to cultural and religious needs.
- Collaboration & Communication: Work closely with families, carers, and allied health and social care professionals to deliver coordinated care. Participate in the assessment and review of care plans and maintain accurate records. Support clients in accessing social and recreational activities, including those provided at the on-site café.
- Operational Responsibilities: Administer medication where required (for community-based roles). Maintain the security of clients’ homes when key holding forms part of the support plan. Take part in supervision, training, and personal development to enhance professional practice. Work flexibly across a 7-day rota to meet the needs of service users.
THE SUCCESSFUL CANDIDATE
The successful candidate will have the following skills and competencies:
- Essential Skills & Attributes: Compassionate, patient, and committed to supporting others to live independently. Reliable and flexible, with a proactive attitude towards high-quality care delivery. Ability to follow structured reablement plans while adapting to individual needs. Strong communication and teamwork skills, with a collaborative approach to care. Comfortable with the physical and emotional aspects of care work, including manual handling.
- Other Requirements: Willingness to undertake relevant training and continuous professional development. Understanding of safeguarding, equality, and confidentiality principles. Enhanced DBS (Adult Workforce and Barred List) required prior to appointment.
