Are you eager to land a brand new role? Our client, based in the South, are looking for a Reablement Assessor to work within the community.
Rate: £23.66 per hour
Purpose: Work within the community based service, to complete strength based assessment following hospital discharge.
Job Responsibilities:
- Undertake assessment for risk, reablement plans, independence and community work
- Deliver short term outcome focused support for timely recoveries.
- Working across multi-agency partners, to develop and review care plans and intervention
- Contribute service improvements and report on outcomes
- Provide guidance to frontline staff
Job Requirements:
- Experience within Adults Social Service
- Level 3 diploma or NVQ required
- Knowledge of relevant legislation
- Enhanced DBS required
