Are you eager to land a brand new role? Our client, based in East London, are looking for a Team Manager to work within the Hospital Discharge Team.
Position: Team Manager
Rate: £43 per hour
Contract Duration: 3–6 Months
Purpose: Managing own caseload of adults discharged from hospital, delivering high quality assessments and supporting individuals across discharge pathways.
Job Responsibilities:
- Assess and support social workers with caseloads of citizens on discharge from hospital across pathways.
- Care Act assessments and support planning post discharge.
- Manage mental capacity and best interest assessments.
- Manage safeguarding concerns in line with procedures.
- Provide duty cover and attend QA panels for funding provision.
- Multi agency working, developing good relationships for individuals and their families.
Job Requirements:
- Must have experience within Adult Social Care and hospital discharge.
- Qualified Social Worker (DipSW, CQSW, CSS, MSc or equivalent).
- Registered with Social Work England.
- Strong knowledge of Care Act, safeguarding and mental capacity.
- Experience completing assessments and managing caseloads independently.
