Are you eager to land a brand new role? Our client, based in East London, are looking for a Team Manager to work within the Hospital Discharge Team.

Position: Team Manager

Rate: £43 per hour

Contract Duration: 3–6 Months

Purpose: Managing own caseload of adults discharged from hospital, delivering high quality assessments and supporting individuals across discharge pathways.

Job Responsibilities:

  • Assess and support social workers with caseloads of citizens on discharge from hospital across pathways.
  • Care Act assessments and support planning post discharge.
  • Manage mental capacity and best interest assessments.
  • Manage safeguarding concerns in line with procedures.
  • Provide duty cover and attend QA panels for funding provision.
  • Multi agency working, developing good relationships for individuals and their families.

Job Requirements:

  • Must have experience within Adult Social Care and hospital discharge.
  • Qualified Social Worker (DipSW, CQSW, CSS, MSc or equivalent).
  • Registered with Social Work England.
  • Strong knowledge of Care Act, safeguarding and mental capacity.
  • Experience completing assessments and managing caseloads independently.
