Are you ready to take on a new leadership role? Our client, based in London, is looking for an Adults Team Manager to work within the Integrated Discharge Service.
Position: Adults Team Manager
Rate: £43.00 per hour
Contract Duration: Ongoing
Purpose: Lead a team to deliver effective hospital discharge planning, ensuring residents are supported to return home safely while promoting independence and wellbeing.
Job Responsibilities:
- Lead and manage a team of social workers and practitioners
- Oversee complex discharge planning and reduce delayed transfers of care
- Ensure all statutory duties are met under the Care Act, Mental Capacity Act and safeguarding
- Manage team performance, caseloads and quality of assessments
- Work closely with multi-disciplinary teams including health professionals
- Ensure high-quality, outcome-focused practice across the service
Job Requirements:
- Extensive experience within Adult Social Care
- Strong background in discharge planning
- Experience managing or supervising staff
- In-depth knowledge of safeguarding and relevant legislation
- Registered with Social Work England (or relevant body)
