Are you an experienced Social Worker passionate about supporting young people as they transition into adulthood? Our client is looking for a Social Worker to join their multidisciplinary Transitions service in the Southwest Region.
Position: Social Worker – Young Adults Transitions
Rate: £32 per hour
Contract Duration: 3-6 Months
Purpose:
- You’ll support young people aged 14–25 as they transition between Children's and Adult Services, helping them develop independence and achieve positive outcomes.
Job Responsibilities:
- Complete support conversations, Care Act assessments and support plans.
- Work alongside young people, carers and a multidisciplinary team to identify appropriate support.
- Help young people develop independent living skills and access community-based opportunities.
- Support the commissioning of services, equipment, assistive technology and other resources where required.
Job Requirements:
- Previous Local Authority Adult Social Care or Transitions experience is essential. Please do not apply if you have not worked within a Local Authority setting.
- Social Work qualification with current Social Work England registration.
- Experience working with young people and/or within Adult Social Care.
- Understanding of transitions between Children's and Adult Services.
