Job Responsibilities:

  • Assist the Continuing Health Care Senior Nurse in developing, coordinating and sustaining a Continuing Health Care model and service to meet the needs of the CTMUHB area.
  • Manage a Nurse Assessor/teams within the complex and continuing care operational teams.
  • Provide extensive knowledge and advice on Continuing Health Care and NHS Funded Nursing Care.
  • Deliver educational programmes to ensure staff have the skills to undertake robust CHC assessments and implement CHC and NHS Funded Nursing Care policies across directorates.
  • Lead a team, undertake day-to-day management, work autonomously and delegate appropriately.
  • Act in the Senior Nurse’s absence for key policy areas as required.

Person Specification:

Essential

  • Qualifications/Knowledge: Registered Nurse with current NMC Registration, Degree in a relevant subject, plus Masters level training or equivalent, Evidence of Continuous Professional Development, Highly specialist clinical knowledge for managing complex cases.
  • Experience: Experience as a Ward Manager/Team Leader (Deputy Team Leader Band 6), Service planning and delivery in Primary, Community and Secondary Care, CHC, NHS Funded Nursing Care, Public Protection and Care Home sectors, Experience in managing a team.
  • Aptitude & Abilities: Clinical supervision skills, Specialist knowledge of CHC/RNCC and complex care; ability to manage internal/external pressures and work with statutory/voluntary bodies, Strong interpersonal, communication, persuasive and negotiating skills; able to support patients and families in distressing situations, Ability to organise, plan and prioritise workload; commitment to collaborative working.

Desirable

  • Welsh Language Skills (Level 3/B2) is desirable
