As part of the Healthy Child Wales Programme, the Specialist Community Public Health Nurse - Health Visiting (SCPHN - HV) will be the named Health Visitor for all children and their families within their caseload from antenatal to 5 years, excluding the unborn child. The Health Visitor (HV) will commence the child’s planning process and, without direct supervision, undertake statutory duties on behalf of the Health Board to assess, monitor, improve and support the wellbeing of children and their families. The post holder will work in partnership with the child’s family and all involved agencies to produce a child’s plan that will be monitored, reviewed and evaluated. The post holder is responsible and accountable for maintaining clinical, staff and information governance.
Main duties of the job include providing a Health Visiting service for a defined caseload of families with children aged 0-5 (including the antenatal period) and ensuring transfer to the appropriate service at the right time. The role provides a child-centred, inclusive, holistic and accessible Public Health programme to children, their families and communities. Within the Healthy Child Wales Programme, a quality assurance framework for current practice and skills such as motivational interviewing are required. The post holder may lead or contribute to strategies addressing identified needs at both individual and community levels. An acuity-based assessment tool will be used to support safe staffing levels and to assist colleagues during fluctuations in workforce and workload. The post holder will participate in Health Visiting Forums, team meetings, working groups and projects relating to future service development and implement changes to routine care. The role also involves leading and delivering the Universal Healthy Child Wales Programme under the Welsh Government framework, adaptable to changes in models of care.
