We are seeking an enthusiastic registered nurse for a Band 6 Tissue Viability Nurse within the Tissue Viability Service. The base will be Prince Charles Hospital and providing Tissue Viability support to Hospitals and primary care facilities, including District Nursing services and care homes within Merthyr and Cynon Localities. There will be a requirement to work across the whole of the Health Board to maintain a Tissue viability service as required to meet the needs and demands of the service. Appointed staff will be fully supported throughout their induction, including any necessary training and a supernumerary period to support their initial "settling in" period and ongoing development.

This isn’t "fluency", just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.

Main duties of the job

  • The Tissue Viability Nurse will act as an expert resource providing specialist advice and support regarding the clinical management of patients presenting with problematic and complex wounds, pressure ulcers and leg ulcers.
  • Working closely with Dermatology, Lymphoedema, Therapy and Vascular services, the post holder will support the Clinical Nurse Specialist in the further development of the Tissue Viability service.
  • Assessment and appropriate management strategies for patients presenting with problematic/complex wounds.
  • Assessment and appropriate management strategies for patients presenting with Pressure Ulcers.
  • Assessment and appropriate management strategies for patients presenting with Leg Ulcers.
  • Educating patients, carers, and multi‑disciplinary professionals regarding the prevention and management of pressure ulcers, the management of problematic, complex wounds, and the management of leg ulcers.
  • Contribute to the development and implementation of policies and procedures within their sphere of responsibility and, where appropriate, actively participate in training and education programmes, patient support groups, research, clinical audit and service evaluation.
  • Act autonomously within Health Board and Professional guidelines, referring to the Clinical Nurse Specialist as required.

Person Specification

  • Essential Registered Nurse & Post Registration qualification to Diploma or Degree level.
  • Specialist qualification in wound management, for example, Principles of Wound Management or willingness to work towards.
  • Evidence of ongoing Continuing Professional Development relevant to the role.
  • Knowledge of the Prevention of Pressure Ulcers Policy and its application in practice.
  • Significant post registration clinical experience within secondary or community setting.
  • Experience of ASSKING framework and management strategies.
  • Experience of wound care management within clinical practice, including the principles of assessment, diagnosis and dressing selection.

Desirable

  • Teaching Qualification.
  • Specialist knowledge of chronic oedema/wound management.
  • Welsh Language Speaking/Listening Skills at Level 3 or above.
  • Secondary or Community Nursing Experience.
