We invite applications for a senior pharmacy technician to lead medicines management for patients receiving Hospital@Home care. The role blends clinical expertise with community care, managing a team of pharmacy technicians and ensuring safe, effective medication support for patients discharged from hospital.
Job Summary
The successful candidate will fully manage pharmacy technicians, develop and deliver medicines‑management support, and coordinate with hospitals, GPs, community pharmacies, and social care providers to improve patient outcomes and independence at home.
Responsibilities
- Provide professional and operational management of pharmacy technicians within the Hospital@Home service.
- Develop, coordinate and deliver medicines‑management support to patients post‑discharge.
- Oversee the assessment of patients’ medication needs and create tailored care plans.
- Act as a key contact and advisor on medicines‑management issues for primary, secondary and social care partners.
- Lead and coordinate activities across health and social care to promote independence and improve outcomes.
- Liaise with hospitals, GPs, community pharmacies and social care providers to ensure safe, effective care.
- Support staff in delivering medicines‑management assistance and promote best practice.
Qualifications
Essential
- NVQ3, BTEC in Pharmaceutical Sciences or equivalent.
- Registered with the GPhC.

- Accredited Checking Technician.
- Medicines Management Technician.
- Quality Improvement qualification.

Experience

- Significant post‑qualification experience as a pharmacy technician.
- Experience in medicines management/near‑patient pharmacy services.
