NHS Arden and Greater East Midlands Commissioning Support Unit (CSU) is seeking an experienced and dedicated CHC Nurse Assessor for an initial 6-month contract. This is a fantastic opportunity for a senior nursing professional to play a vital role in our Continuing Healthcare team, ensuring the delivery of high-quality, patient-centred assessments in line with the National Framework.
About the Role:
As a CHC Nurse Assessor, you will be responsible for facilitating multi-disciplinary assessments and patient reviews for NHS Continuing Healthcare. You will be a key clinical professional, ensuring that complex patient needs are thoroughly evaluated and that decisions are made in accordance with standard operating procedures and national guidance.
This is a patient-facing role requiring daily travel across the Northamptonshire area to conduct face-to-face assessments.
Key Responsibilities:
Facilitate multi-disciplinary assessments and patient reviews in line with the National Framework for NHS Continuing Healthcare.
Manage a complex caseload, ensuring services are delivered to maximise patient safety and adhere to all guidance.
Be responsible for the accurate and legible recording of all patient information and ensure data systems are maintained.
Identify and support the arrangement of services required for individuals who qualify for NHS Continuing Healthcare.
Undertake investigations into eligibility following an individual challenge or dispute.
Act as a Duty Clinician as required and provide cover for other practitioners.
Maintain constructive relationships with a broad range of internal and external stakeholders, including clinicians, social care, and other partner agencies.
Communicate complex information clearly and professionally, providing specialist advice on CHC matters.
Ensure compliance with the NMC Code of Professional Conduct, Data Protection Act, and all statutory and mandatory training.
The Ideal Candidate Will Have:
A registered nursing qualification (RNLD, RMN, or RGN) with current NMC registration.
Substantial and proven experience working within NHS Continuing Healthcare, including conducting complex assessments and interpreting evidence.
Expert knowledge of the NHS CHC National Framework, safeguarding processes, and the Mental Capacity Act.
The ability to analyse complex information, make difficult, contentious decisions, and recommend appropriate courses of action.
Excellent interpersonal and communication skills, with the ability to influence and persuade in challenging situations.
A professional, calm, and efficient manner, with strong organisational and problem-solving skills.
The ability to work autonomously, prioritise a demanding workload, and meet tight deadlines.
A full driving licence and access to a vehicle for daily travel across Northamptonshire.
