We are seeking a motivated and dedicated individual committed to delivering high‑quality, person‑centred support and working collaboratively as part of an integrated team in Stockton‑on‑Tees.
Responsibilities
- Provide essential support to a busy social work team ensuring adults across Stockton-on-Tees receive effective assessments, care management and timely reviews.
- Work within current legislation, government guidance, and departmental policies to support the delivery of safe and responsive services.
- Administer assessments and develop support plans with a focus on accurate and timely record‑keeping.
- Maintain excellent communication skills—both written and verbal—building positive relationships and working effectively as part of a team.

This part‑time role is 30 hours per week with a salary of £25,995 – £27,323.
