Responsibilities
- Provide licensed practical nursing care to patients following care plans.
- Assess patient needs and document observations accurately for care teams.
- Administer medications and monitor for adverse reactions per protocols.
- Adhere to regulatory standards and company policies at all times.
- Coordinate with multi-disciplinary teams to ensure timely patient progress.
- Maintain accurate, up-to-date patient records and eligibility for services.
- Meet productivity targets and contribute to pipeline-based performance metrics.
- Support quality improvement initiatives to increase patient outcomes.
- Educate patients and families on care plans and post-discharge steps.
- Maintain a high-visibility work ethic by documenting daily activities and outcomes.
- Assist with onboarding and mentoring of new staff to foster growth.
Experience / Qualifications
- Valid LPN license in applicable state.
- Experience with medication administration and adverse reaction monitoring.
- Ability to accurately document observations in electronic health records.
- Current CPR/ACLS certification preferred or required.
- Familiarity with care plans and regulatory standards (e.g., HIPAA, OSHA).
- Strong collaboration with multidisciplinary teams to coordinate care.
- Commitment to quality improvement initiatives and patient outcomes.
- Demonstrated productivity and contribution to pipeline-based targets.
- Excellent patient and family education skills for discharge planning.
- Willingness to mentor and onboard new staff to support growth.
- On-site availability for contract role with high visibility and daily activity documentation.
