NHS South West London Integrated Care Board (ICB) is seeking experienced and qualified Nurse Assessors to join the Croydon Continuing Care Team. This role offers competitive NHS Band 7 salary and full support for UK Skilled Worker visa sponsorship.
About the Role
As a Nurse Assessor, you will:
- Lead and coordinate Multidisciplinary Team (MDT) assessments to determine eligibility for NHS Continuing Healthcare (CHC) and Funded Nursing Care (FNC)
- Conduct assessments in a variety of settings including hospitals, care homes, and patient homes
- Work collaboratively with patients, families, care providers, and health/social care professionals
- Maintain accurate records in compliance with the National Framework for CHC (revised 2022)
- Promote independence, individual choice, and care quality across all settings
You will work flexibly across the five boroughs covered by the South West London ICB: Croydon, Merton, Kingston, Richmond, Sutton, and Wandsworth.
Key Responsibilities
- Manage referrals and co-ordinate assessments
- Use national tools to assess CHC/FNC eligibility
- Communicate decisions to patients, families, and professionals
- Ensure proper care packages and review processes are in place
- Support continuity and transparency of the assessment process
Who We’re Looking For
Essential Qualifications & Experience:
- NMC registered nurse
- Degree or equivalent in health or social care
- Experience working in complex, multi-disciplinary settings
- Knowledge of the National Framework for CHC
- Community, learning disabilities, or mental health nursing experience
Other Requirements:
- Ability to travel across the boroughs and occasionally regionally or nationally
- Strong communication and documentation skills
- Committed to NHS values and inclusivity
- Must pass a DBS (Disclosure and Barring Service) check
UK Registration Required
Applicants must have or be eligible for UK NMC registration.
