NHS North West London Integrated Care Board (ICB) is hiring Clinical Assessors to join its Continuing Healthcare (CHC) team. This is a full-time, permanent opportunity for experienced nurses or allied health professionals with a passion for improving adult health outcomes through structured assessments and care planning.

You will play a key role in leading operational and clinical activities related to CHC and Funded Nursing Care (FNC), ensuring compliance with national frameworks while working collaboratively with partner organisations and multidisciplinary teams.

Key Responsibilities

  • Conduct face-to-face assessments and reviews for CHC and FNC
  • Support eligibility decisions in line with NHS CHC National Framework
  • Prepare comprehensive clinical reports for CHC panel reviews
  • Collaborate with local authorities, care providers, and families
  • Promote a person-centred approach to care planning
  • Contribute to case management and discharge coordination
  • Support patients in accessing Personal Health Budgets
  • Maintain accurate records and data for audit and performance monitoring
  • Apply safeguarding principles and Mental Capacity Act guidance
  • Stay current with NHS CHC legislation, policy, and service improvement goals

Candidate Requirements

Essential

  • NMC or HCPC registered professional (e.g. RGN, RMN, RNLD, AHP)
  • Evidence of Continuing Professional Development
  • Solid understanding of NHS CHC National Framework
  • Case management experience in acute, community or social care settings
  • Strong communication, analytical, and negotiation skills
  • Ability to work independently in complex and emotionally sensitive situations
  • Familiarity with mental capacity assessments and safeguarding procedures
  • Knowledge of health commissioning and care provision

Desirable

  • Experience in NHS Continuing Healthcare assessments
  • Understanding of adult social care markets and procurement processes
  • Experience supporting discharges or coordinating multi-agency care
  • Member of a professional body or special interest group
