Avon and Wiltshire Mental Health Partnership NHS Trust (AWP) seeks an STR worker to support timely discharge from an inpatient mental health ward. You will assess barriers, assist with housing, finances and recovery, and coordinate with the MDT to identify care pathways out of hospital for service users.

The role requires working with service users with complex needs, including homelessness and substance misuse, under supervision of the Ward Manager.
