Responsibilities

The post-holder is responsible for the management of a defined caseload and is charged with the development and delivery of programmes of care. This involves comprehensive assessment, planning, provision and evaluation of patient care needs. The post-holder manages and supports junior staff by prioritising and delegating work, monitors and ensures safe working practices, and acts as a clinical leader within the integrated teams. They further the team’s understanding of risk management plans, facilitate preparedness for foreseeable events and promote self-care approaches. The role requires high clinical judgement and autonomous decision-making across the health and social care economy.

  • Specialist Practitioner Qualification
  • Extended Nursing skills
  • Car driver / access to a car

  • Ability to organise workload and work autonomously and within a team
