Were lookingfor an enthusiastic Wellbeing CareCo-ordinator to join our friendly team and be the first point of contact for patients needingproactive well-being support across five local practices.
You willwork closely with GP teams, Care Coordinators, social prescribers, healthprofessionals, and community groups to help patients get the right supportfor their needs.
You\’llalso be involved in local health projects that focus on helping people who may find itharder to access services.
Workingalongside our supportive Wellbeing Team, youll help individuals and groupsby offering personalised care guiding them to make informed choices thatsupport their health and wellbeing goals.
*This is position working is for 32 hours per week working 4 or 5 days days*

Main duties of the job
Key responsibilities will include:
In this varied role, youll support patients toaccess the care and services they need while promoting health and wellbeingacross the PCN.
You will coordinate new referrals to the WellbeingTeam, offer advice and signposting, and work one-to-one with patients to helpthem take an active role in their care. The role also includes supportinggroup programmes for long-term conditions, engaging in community outreach,and promoting the Wellbeing Team across GP practices and communities. You willwork closely with GPs, social prescribers, health coaches, dietitians, andsafeguarding leads.
We are looking for a reliable, people-focused personwith strong communication skills, able to manage a varied workload, recordpatient interactions accurately, and collaborate effectively. Anunderstanding of health inequalities/ interest in community healthinitiatives is essential, with a willingness to learn and develop throughtraining and supervision.

Job responsibilities
The Wellbeing Team work with individuals in need of proactive support to encourage independence, a health lifestyle, mental wellbeing and social connectivity. They work with General Practices and their communities where there are identified health inequalities to engage and support people in accessing health and community services
The Wellbeing Care Coordinator (WBCC) has a key care coordination role within the N&W PCN Wellbeing Team in supporting people to live independent, healthy and positive lives. The role will:
- Act as a first point of contact for all new referrals to the Wellbeing Team and the single point of access for staff and patients supporting referral issues, early signposting advice and queries.
- Involve working one to one with patients who need extra support and time, helping them to be actively involved in their care and directing them to the most appropriate services based on what is important to them.
- Support the wider Wellbeing and Primary care team in delivering specific health inequality initiatives across the PCN. This could include promoting uptake to screening services, coordinating self-management group based programmes to manage long term conditions such as BP, diabetes and community outreach initiatives working alongside public health teams
- Actively engage with practices to promote the activities and support available from the Wellbeing Team across all practices
- Work closely with and understand the different roles within the wider Wellbeing team (Social prescribers, Health and Wellbeing coaches, dieticians, Care Home and Safeguarding coordinators.) and General Practice based staff.
There will be a need to work agilely- within the local community and in practices as well as remotely depending on the requirements of the service.

Qualifications
- GCSE English or equivalent level.
- GCSE Mathematics or equivalent level.
- ECDL or equivalent level of keyboard/IT skills.
Skills
- Excellent interpersonal communication skills both written and verbal
- Ability to support people in a way that inspires trust and confidence, motivating others to reach their potential
- Able to provide culturally sensitive, person-centred care that respects diverse backgrounds, lifestyles, and beliefs.
- Ability to manage and prioritise own work to meet deadlines.
- Ability to work effectively as part of a team.
- Good level of accuracy and attention to detail.
- Able to communicate health information in a clear, simple, and easy-to-understand way to support informed decisions.

- Marketing and communication skills, including creating posters, leaflets, and digital content to promote services.
Other
- Understanding of confidentiality and Data Protection.
- Required to travel to work from practices and other community locations in order to carry out work across the N&W PCN
Experience
- Working knowledge of Microsoft office Word, Excel, Outlook and PowerPoint and using video calling software e.g. Microsoft teams.
- Experience of using IT systems such as EMIS or similar
- Experience of working in a health or social care setting (or related sector e.g. VCS, housing, employment )
- Experience of partnership/collaborative working and of building relationships across a variety of organisations
- Knowledge of the core concepts and principles of personalised care
- Knowledge of, or experience in, community development approaches, working in community outreach settings
- Understand how social, economic, and environmental factors affect health and wellbeing for individuals, families, and communities.
- Experience of minute-taking within an MDT environment or multiagency setting
- Completed a two day PCI accredited care co-ordination training course or be willing to complete one prior to taking referrals
- Experience of providing one to one and/or group support
