Exciting job opportunity for an Advanced Care Practitioner (Nurse Practitioner/Paramedic) at Belmont Medical Centre, Hereford (WBC PCN). We are seeking a qualified ACP with experience in general practice or urgent care settings, confident in managing a broad range of clinical presentations independently. This position is for approximately 28 hours per week, Monday to Friday. Closing date is Monday 2nd March 2025.

Main duties of the job:

• You’ll play a vital role in offering expert clinical care, managing both acute and chronic conditions.

• Hours of work Monday to Friday triaging and seeing patients, reviewing and actioning laboratory results, signing prescriptions (if you are an independent prescriber), and supporting urgent clinical queries.

Job responsibilities:

• To deliver a high standard of patient care as an Advanced Care Practitioner - ACP, in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.
• To manage a clinical caseload, dealing with presenting patients' needs in a primary care setting.
• Telephone triage and seeing patients, reviewing and actioning laboratory results, signing prescriptions if you are an independent prescriber.

Primary Key Responsibilities:

• To manage patients presenting with a range of acute and chronic medical conditions, including undifferentiated undiagnosed conditions, providing subject matter expert advice.
• To identify red flags and underlying serious pathology and take appropriate action.
• To use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
• To implement and evaluate individual specialised treatment plans for chronic disease patients.
• To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients' health.
• To provide advanced, specialist medical care to patients as required in accordance with clinical based evidence, NICE and the NSF.
• To assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the primary care team.
• To process and interpret pathology and other test results as required in a timely and clinically appropriate manner.
• To maintain accurate and up-to-date clinical records in line with practice and regulatory standards.
• To ensure SNOMED CT codes are used effectively.
• To maintain chronic disease registers.
• To chaperone patients where necessary.
• To support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required.
• To contribute to practice targets (QOF, etc.), PCN targets and service requirements complying with local and regional guidance.
• To complete all relevant training in order to provide multi-professional clinical practices and CPD supervision to other roles within primary care.
• To liaise with external services/agencies to ensure the patient is supported appropriately.
• To delegate clinical responsibilities appropriately ensuring safe practice.
• To support the clinical team with all safeguarding matters in accordance with local and national policies.

Person Specification:

Knowledge: Understanding and knowledge of policy developments related to the delivery of primary care services including General Practice, the GMS contract, Clinical Governance, Quality & Outcomes Framework. Understanding of evidence based practice. Knowledge of national standards that inform practice. Understanding of their accountability arising from Codes of Professional Conduct and medico-legal aspects of the ACP role. Understanding of equal opportunity and diversity issues.

Experience: Minimum of 5 years post registration experience. Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and ongoing competence in all aspects of the NP role. Experience of dealing with patients presenting with undifferentiated, undiagnosed problems. Experience of prescribing safe, effective and appropriate medication as defined by current legislative framework. Experience in management of long term conditions.

Skills: Ability to assess and manage patient risk effectively and safely. Well developed word processing/data collection/IT skills. Excellent interpersonal, verbal and written communication skills. Time management and ability to prioritise workload. Able to analyse data and information, drawing out implications for the individual patient/impact on care plan.

Qualifications: Registered ACP/NP/Paramedic. Recognised ACP qualification at minimum of degree level or equivalent. Teaching/Mentoring experience and/or qualification.

£46,000 to £55,000 a year pro rata - dependent on experience.
