Are you looking for an exciting role providing medical oversight for community multi-disciplinary teams working to prevent unnecessary hospital admissions for people whose needs are best met in their own home? We are looking for regular experienced GPs to join our integrated team working flexibly on a Monday and/or Tuesday. The hours available include 08:00-20:00 or 08:00-14:00 and 14:00-20:00. The role will be based at Nelson House in Hereford. This role would ideally suit GPs who would welcome the time to unpick complex medical histories and support proactive care planning; those with an interest in frailty; and those who enjoy supporting the learning and development of advanced practice colleagues.
Salary: In hours rate from £46.12ph - 47.75ph, Out of Hours rate (18:00-20:00 & weekends) £55.34ph - £57.65ph. Interviews are expected to be held on week commencing 9th March 2026.
Main duties:
- The GP will provide telephone and video support to the community ACPs, nurses and therapists assessing patients in the community.
- The GP will support patients who are managed in a GP-run virtual ward and will also support paramedics and the ambulance service for patients who do not need conveyance to hospital but may still need some medical intervention.

Job responsibilities:
- To provide a full general practice service across different settings according to the portfolio role of the GP and the needs of the service.
- The post holder will be responsible for seeing patients in an identical manner to regular general practice work, taking responsibility for a patient’s care and only referring back to their own GP where appropriate, consistent with ensuring every contact counts.
- Unpicking complex medical histories and support proactive care planning; those with an interest in frailty; and those who enjoy supporting the learning and development of advanced practice colleagues.
- To assist in development of the service, with incident reporting and suggestions for improvements through the appropriate system.
Person Specification:
Essential:- Work autonomously and collaboratively, recognising when referrals are required.
- Excellent IT skills- computer and keyboard literate.
- Excellent communication skills, both written and verbal.
- Excellent time management skills and the ability to deliver work to tight deadlines.
- Ability to prioritise workload.
- Relay clinical information and maintain high quality accurate, comprehensive consultation records.
- Qualified General Practitioner.
- A demonstrable commitment to professional development.
- Safeguarding adults.
- In date CPR training (within 18 months)
- Current clean driving license.
- Experience of working in Primary Care.
- Experience of working with patients.
- Experience of working autonomously.
- An understanding, acceptance and adherence to the need for strict confidentiality.
- Knowledge and experience of using EMIS Web.
- Experience of working under pressure.
- Experience of healthcare in Herefordshire.
