We are seeking an experienced Lead Clinical Pharmacist to develop and expand our Primary Care Network pharmacy team. The post holder will work across the two GP practices delivering advanced clinical medicines optimisation, structured medication reviews, and medicines safety leadership. This is a senior, patient-facing role requiring autonomous practice as an independent prescriber, with responsibility for complex patients, service development, quality improvement and leadership within the multidisciplinary team.
Main duties of the job:
- Deliver face-to-face and remote structured medication reviews, focusing on polypharmacy multimorbidity care home residents and vulnerable patients.
- Act as an independent prescriber, implementing prescribing changes and ordering appropriate monitoring.
- Lead repeat prescribing systems, reauthorisations and medicines optimisation.
