The Tasmanian Health Service is seeking an experienced Clinical Nurse Consultant to join the Care@home program in the role of Cardiac Rehabilitation Navigator. This role provides statewide clinical leadership and coordination to support a consistent, equitable and person‑centred cardiac rehabilitation service across Tasmania, with a strong focus on virtual and hybrid models of care.

The Cardiac Rehabilitation Navigator works at a system level to support the implementation and ongoing operation of a statewide cardiac rehabilitation model of care. The role focuses on clinical navigation, referral and triage coordination, pathway optimisation and service integration, rather than routine direct clinical care.

Key responsibilities include:

  • Acting as a central clinical navigation point for cardiac rehabilitation pathways across acute, outpatient, primary care and virtual services
  • Supporting consistent statewide referral, triage, prioritisation and escalation processes
  • Improving access, flow and participation for people requiring cardiac rehabilitation, particularly those in rural, remote and priority populations
  • Providing expert clinical advice to clinicians and services regarding pathway eligibility, care options and coordination
  • Supporting the embedding of virtual and hybrid cardiac rehabilitation models, including use of digital platforms and alternative delivery options
  • Building strong collaborative relationships with cardiology services, cardiac rehabilitation providers, primary care and virtual care teams
  • Contributing to service integration, quality improvement, outcomes monitoring and continuous improvement initiatives

You will need:

  • Advanced clinical experience in cardiac care, chronic disease, virtual care or system‑level service coordination
  • Demonstrated expertise in clinical leadership, navigation and working across service boundaries
  • Strong communication, stakeholder engagement and problem‑solving skills
  • Experience supporting change, pathway redesign or service improvement
  • A commitment to equitable, evidence‑based and person‑centred care

Note: Please refer to the Statement of Duties for all the selection criteria with the above responsibilities forming the core functional requirements of the Cardiac Rehabilitation Navigator role to address in your application.

Details of appointment:

  • Fixed term, full time, day worker position, working 76 hours per fortnight, commencing as soon as possible for a period of up to thirteen (13) months.
  • Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.
  • In addition, the following allowances calculated on the salaried incremental point may be available: Post Graduate Allowance of up to 10% based on qualification type.
