The Role

You will be based in Hull and will deliver high quality support to individuals who have a learning disability and /or autism. The support will be provided in our accommodation, as well as in the community and other appropriate settings. You will have an allocated caseload and be responsible for completing risk assessments, support plans and other paperwork. You will play an instrumental part in empowering individuals to live more independently, co-ordinating support with other key agencies, attending multi-disciplinary meetings.

The support will include but not be limited to, assisting with daily living skills including shopping, attending appointments, accessing community resources, health care and accessing and dealing with benefits to maintain a safe and comfortable living environment. This service is run over 7 days a week.

Responsibilities include:

  • Deliver 1:1 support to clients in line with the goals set out in their support plan
  • Liaise with the others involved in an individual’s care regarding progress towards set goals
  • Keep up to date, accurate and professional recordings of support delivery
  • Liaise with external agencies and professionals involved in clients support
  • Work flexibly as part of the staff rota to ensure safe staffing levels are met (five days out of seven)
  • Encourage and support service users to take part in activities
  • Encourage service users to contribute fully to their care and support plan
  • Support service users with personal administration, form filling and making benefit claims as necessary

Qualifications / Requirements:

  • A valid UK driving license and access to own roadworthy vehicle. Business insurance will be required.
  • Enhanced DBS check.
  • Right to work in the UK.
  • Experience working with mental health
  • An understanding of relevant legislation relating to vulnerable adults
  • Experience of working in partnership with Local Authorities and Health/Social Care professionals
  • NVQ level 2 in Health and Social Care is preferred
  • Intermediate IT skills which must include Outlook, Word and Excel
  • Effective and clear written communication skills in report writing, risk assessments and form completion
  • Excellent time-management skills
