We’re looking for a number of experienced and compassionate Recovery Case Workers to join our dedicated team in Sheffield. Working closely with our Service Manager and Recovery Team Leader, you will play a key role in supporting our service at St Bart's. As a proactive Recovery Case Worker, you will have an empathetic and person‑centred approach, ensuring service users receive the dedicated support they require.
The successful candidate will have experience working with individuals with multiple and complex needs, and will be confident in delivering support both 1:1 and in group settings. You will have a strong understanding of tenancy sustainment, risk management, and support planning. Above all, we are looking for someone who is empathetic, organised, and committed to helping people achieve their potential and move towards independent living.
General Tasks- Supporting the delivery of high quality 1:1 and group activities in people's homes and in the community
- Manage and advocate for a caseload of service users who have needs around mental health, learning disabilities, multiple disadvantages
- Develop and maintain effective working relationships with relevant agencies to provide a joined-up network of support for clients
- Keep and update accurate records on the case management system
- A relevant qualification in mental health, learning disabilities, health and social care, or similar
- Experience of supporting people with multiple complex needs, particularly with mental health and learning disabilities
- Experience of building successful professional relationships both with service users, colleagues and professional networks
- Knowledge and understanding of PIE (Psychologically Informed Environments) and TIP (Trauma Informed Practice)
- Strong organisational and communication skills
