Main Purpose

To deliver the Mental Health Pathways program working closely with our NHS colleagues to support timely discharges from local NHS wards, reducing the prevalence of people being discharged from wards homeless and prevent delayed discharges in accordance with Target Housing policies and procedures.

Accountabilities

  • Support people to source and secure accommodation from a range of housing providers including supported living and housing private landlords, social and local authority.
  • Provide advice and guidance as to the range of options available.
