We’re seeking a dynamic and compassionate Nurse Practitioner to support youth aged 12–25 within an integrated health and wellness hub. This role offers a unique opportunity to provide rapid, barrier-free access to primary care for young people navigating mental health, substance use, and wellness challenges.
Job Details
- Location: Scarborough, ON (Hybrid work model)
- Hours: 21 hours per week
- Compensation: $48.35–$68.48/hour
- Employment Type: Permanent, Part-Time
Key Responsibilities
- Deliver comprehensive episodic primary care for youth and young adults.
- Perform full assessments, including health history and psychosocial factors.
- Diagnose and manage acute and stable chronic conditions.
- Prescribe medications and order diagnostic tests within NP scope.
- Provide health education and integrate psychotherapeutic approaches as needed.
- Collaborate with a multidisciplinary team (counsellors, peer workers, navigators).
- Participate in program development, quality improvement, and community outreach.
- Provide mentorship and contribute to knowledge-sharing within the team.
Qualifications
- Master’s degree in Nursing (Primary Health Care NP specialization).
- Registration in good standing with the College of Nurses of Ontario (Extended Class).
- Experience working with youth and/or in mental health, addictions, or primary care.
- Knowledge of trauma-informed, harm reduction, and anti-oppressive practices.
- Strong decision-making, communication, and leadership skills.
- Ability to work hybrid (on-site and virtual) and flexible hours, including evenings.
