Key Responsibilities

Assessments & Reviews: Conduct strengths-based assessments, care reviews, and mental capacity assessments (MCA) under the Care Act 2014.

Safeguarding: Lead and contribute to adult safeguarding inquiries to protect vulnerable individuals from harm or neglect.

Hospital Discharge & Prevention: Work alongside NHS multi-disciplinary teams to support safe, timely hospital discharges and prevent unnecessary admissions.

Support Planning: Co-produce outcome-focused care plans with adults, carers, and external healthcare providers.

Duty & Triage: Cover duty desks and manage urgent intake caseloads as required by local service demands.
Requirements

Qualifications: Degree in Social Work (BA, BSc, MA, DipSW, or CQSW).

Registration: Active registration with Social Work England.

Experience: Minimum 1–2 years post-qualifying experience within Adult Social Care (e.g., Community, Duty & Assessment, Hospital Discharge, or Safeguarding).

Compliance: Enhanced DBS on the Update Service (or willing to obtain one) and two professional references covering the last 2 years.

Mobility: Valid UK driver’s licence and access to a reliable vehicle for community visits across your preferred county.
