The Role
An exciting opportunity has arisen to join the Home and Community Support Team as a Patch Co-ordinator. We are looking for a highly motivated individual to enhance our existing team and further their development within a Team that is currently looking at new ways to support individuals to optimise their independence by creating an individualised support plan that is reviewed weekly and joint working with our colleagues to have a multi-disciplinary approach to secure the best possible outcomes for individuals requiring support. Shift times for this post are between 07.00 22.00 over seven days a week, which includes some weekend and bank Holiday working.
Key Responsibilities
- Developing a plan of care that incorporates the individuals’ goals and aspirations to maximise a person’s level of independent living.
- Risk assessments for the individual and staff providing support.
- Collaboration with multiple professionals to ensure the best outcomes are achieved for the individual.
- Responsible for a team of support workers to ensure high standards are maintained.
- Be accountable for your own professional development and practice.
- Problem solving using available resources.
About You

You will need to have a driving licence and access to a vehicle and ideally the applicant should have a health or social care background. A level three care certificate is desirable.
