Tameside Council is seeking a Family Hub Outreach Worker to deliver the Best Start in Life vision across our Family Hubs. The role focuses on outreach to families with children aged 0-19 and up to 25 for SEND, providing joined-up support and early intervention within communities.
Working with health, education, early help and the voluntary sector, you will build trusted relationships, help families access services, and ensure early identification of needs while safeguarding and promoting welfare.
