The Role

This is an exciting opportunity to join Tameside Adult's Services as a Social Worker. The role involves leading support for adults to live well at home, managing a caseload, conducting Care Act Assessments, Support Plans, Mental Capacity Assessments, and Best Interest Decisions, and signposting to internal and external services where necessary.

Responsibilities

  • Support adults to live well at home.
  • Manage a caseload of adults.
  • Conduct Care Act Assessments, Support Plans, Mental Capacity Assessments, and Best Interest Decisions.
  • Signpost to internal and external services to enhance wellbeing and independence.
  • Collaborate with other disciplines and local health and social care services to coordinate responses.

Qualifications and Experience

  • Excellent communication skills.
  • Ability to build effective professional relationships with other disciplines and local health and social care services.
  • Experience of working with adults and previous employment in a similar position would be desirable.
  • Hold a current valid driving licence and have access to a vehicle for work use.

Professional Bar Criteria

  1. Relevant professional qualification for the job role at Level 6 or above, AND
  2. Minimum of 2 years post-qualification experience in the role, AND
  3. Maintained membership of a regulated body where required, AND
  4. Professional standards practised to the highest expected level.
