The Role
This is an exciting opportunity to join Tameside Adult's Services as a Social Worker. The role involves leading support for adults to live well at home, managing a caseload, conducting Care Act Assessments, Support Plans, Mental Capacity Assessments, and Best Interest Decisions, and signposting to internal and external services where necessary.
Responsibilities
- Support adults to live well at home.
- Manage a caseload of adults.
- Conduct Care Act Assessments, Support Plans, Mental Capacity Assessments, and Best Interest Decisions.
- Signpost to internal and external services to enhance wellbeing and independence.
- Collaborate with other disciplines and local health and social care services to coordinate responses.
Qualifications and Experience
- Excellent communication skills.
- Ability to build effective professional relationships with other disciplines and local health and social care services.
- Experience of working with adults and previous employment in a similar position would be desirable.
- Hold a current valid driving licence and have access to a vehicle for work use.

Professional Bar Criteria
- Relevant professional qualification for the job role at Level 6 or above, AND
- Minimum of 2 years post-qualification experience in the role, AND
- Maintained membership of a regulated body where required, AND
- Professional standards practised to the highest expected level.
