Cancer Care Coordinator – 20.5 hours – Fixed term 9 months
The post holder will support the implementation of the Personalised Care Agenda for people affected by or living with Cancer, including:
- Working closely with the cancer nurse specialists by: assisting providing Holistic Needs Assessments and producing associated care plans
- supporting the implementation and administration of Treatment Summaries
- organising health and well-being events
- Providing a single point of access, including rapid re-entry into the system for those people identified as having urgent or specialist needs
- Communicating effectively (daily and directly, face-to-face and telephone) with a range of individuals on various levels, e.g. patients, relatives, carers, other health / social care professionals and community workers
- Providing general information and support about cancer and cancer services to enable navigation of health and social care and to make choices that are best for the individual’s cancer and their life
- Promoting equity for patients
The post holder will be expected to work with all grades of medical and nursing staff and wider health / social care and community teams (including third sector and charity).
- Triage telephone and email enquiries, using a risk assessment framework, initiating an appropriate response according to protocols and individual pathways (referring complex decisions to the Team for assessment and review); some requiring tact or diplomacy, prompt action and communication
- Complete all the administration required in relation to the electronic Holistic Needs Assessments and care planning
- Complete any follow up work generated by holistic needs care planning, e.g. making onward referrals
- Organise and prioritise the designated workload according to identified needs
- Co-ordinate the handover with other teams to facilitate safe, effective seamless transition of care between services
- Assist with the planning and delivery of Health and Well-being events
- Communication and signpost to appropriate needs related information and support and encourage self-management where appropriate
- Guide people through the use of self-assessment resources
- Evaluate outcomes of care delivery with the registered practitioner
- Ensure patients/carers know how to contact the service when their condition / needs have changed, including out of hours
- Act as an advocate and facilitator to resolve issues that may be perceived as barriers to care

The role is to support patients and their families following a cancer diagnosis. You will work alongside the Cancer Clinical Nurse Specialists to support them in ensuring that the patient has access to personalised care and support planning.
