Salary:£48,000 £64,000 per annum, dependent on experience

Key Responsibilities:

Clinical Assessment & Diagnosis:Conduct comprehensive assessments, including history-taking, physical examinations, and diagnostic tests, to formulate accurate diagnoses and treatment plans.

Patient Management:Prescribe medications (as an independent prescriber), initiate and manage treatment regimens, and provide follow-up care.

Chronic Disease Management:Lead clinics for long-term conditions such as diabetes, hypertension, and asthma, implementing personalized care plans.

Clinical Leadership:Provide mentorship and support to junior nursing staff, fostering a culture of continuous professional development.

Quality Improvement:Participate in clinical audits, research initiatives, and service evaluations to enhance care quality and patient safety.

Collaboration:Work closely with GPs, consultants, and other healthcare professionals to deliver integrated care pathways.

Health Promotion:Develop and deliver health education and promotion programs to patients and the wider community.

Record Keeping:Maintain accurate and up-to-date patient records in accordance with legal and professional standards.

Registered Nurse with a valid NMC PIN.

Masters degree in Advanced Clinical Practice (or working towards).

Independent Prescriber status (V300).

Minimum of 2 years\’ post-registration experience in a relevant clinical setting.

Demonstrated competence in advanced clinical assessment and decision-making.
