About the Role
Liquid Personnel is seeking a dedicated and skilled Adult Social Worker to join our Locality Team. The successful candidate will provide support and services to adults within the community, helping them to achieve their full potential and improve their quality of life. This role involves assessment, planning, and intervention to address the needs of individuals and their families.
Responsibilities
- Report to the Team Manager / Senior Practitioner.
- Work in partnerships with customers and their carers to provide personalised support and achieve the individuals’ goals.
- Complete assessments, reassessments, support planning and/or review functions under the National Health Service and Community Care Act.
- Provide relevant information, advice and support to customers and their carers where the customer does not meet the Council's eligibility criteria.
- Ensure that all work is risk based incorporating safeguarding at all times.
