This is an opportunity for a dedicated Registered Nurse Case Manager to join our client and make a difference in the lives of patients in hospice care. You will be part of a supportive team, providing essential care and coordination for patients and their families.
Key Responsibilities
- Manage patient care in hospice settings
- Coordinate with healthcare teams
- Provide leadership and support to nursing staff
Territory/Driving Expectation: Greensburg and 15-25 miles +/- in a radius surrounding the community
Schedule (Including any on call/weekends): 6 units daily; 30 units a week; 8 AM – 5 PM, Mon-Fri schedule. 1 weekend a month; 1 evening of on-call per week.
Travel: Local travel required covering multiple counties.
Qualifications Required
- High School/GED
- Hospice (1+ years)
- Management (1+ years)
- 1+ years of experience in ANY of the following: Home Health Care, Epic Home Health, Home Health
- Valid Registered Nurse (RN) in the state of Pennsylvania (PA)
- ALL of the following valid licenses/certifications:
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
