Somerset NHS Foundation Trust is seeking a Liaison and Diversion Support, Time and Recovery Worker (STR) to join the LADS team. Based at Bridgwater Police Custody with rotation to the LADS community access service, you will assess vulnerable individuals and coordinate referrals to health and social care services.
You will work with multi‑disciplinary teams, maintain accurate clinical records, and provide advice and encouragement to service users.
