Tailored Transitions Ltd are looking for a passionate and inspiring team leader to join our homes, supporting young adults with complex medical, physical and learning disabilities to live their best lives.
You will be responsible for assisting management with the day to day running of the home, supporting and mentoring team members, liaising with family and health care professionals, while ensuring a 5-star level of care is provided on a daily basis.
The role also includes supporting with invasive personal care at times throughout the day, management of medication, daily record keeping, accessing the community, socialising with friends and family.
Our individuals have very busy social lives, we are looking for people who are caring and energetic and can continue to help them live life to the full. Activities involve hydro therapy, sailing, cinema, dining out, shopping, music therapy, horse riding, bike riding, themed nights, parties, concerts, holidays and more.
Role: Full time - 42 hours per week Two week rolling Rota including On-call duties, we are looking for a team player who can be flexible in their working hours and adapt to changes within the company. You will be expected to work every other weekend.
Candidate requirements:
- Previous experience in similar role
- Minimum of 2-year experience in care
- Level 3 Diploma in health and social care
- Full manual UK driving license
- Good oral and written English
