Remote Clinical Case Manager – Rehabilitation This opportunity is working for a Rehab and Injury case management specialist company They are one of the UK’s largest independent providers of rehabilitation case management services. Typical patients would have sustained a Brain injury, Spinal cord injury or Major Trauma Job locations Available: London & Birmingham – although this role is remote it will involve some field-based assessments and meetings Salary Package up to £45,000 Monday – Friday 9 am – 5 pm MUST BE A NURSE, OCCUPATIONAL THERAPIST, OR PHYSIOTHERAPIST – WITH , Neuro, Trauma, Brain Injury or Spinal injury and have dealt with patient rehab Duties: – Carrying out initial assessments – Support Clients to identify & achieve full potential or focussed goal – The Aim would be to facilitate a return to work or the best possible functionality – Set out a road map for recovery – Identify any potential barriers to recovery – Point of Contact to client/patient – working in conjunction with family members, employers, Medical professionals & Insurance organisations – Implementing & Monitoring. Must haves to apply: · UK experience and registration with NMC or HCPC · Right to work in the UK – They cannot offer working visas · Must have own car with valid UK driver’s license
