Overview: We are looking for a compassionate, empathetic and motivated Outreach Support Worker to join our team. You will be supporting individuals experiencing both mental health challenges and substance misuse difficulties. Referral based, the role involves meeting people in their homes for an initial assessment to complete a risk assessment plan and a support plan to improve the quality of their lives. This means you will be mainly lone-working after training is provided. An understanding of how mental health illness and addiction are co-related and being non-judgemental is very important in this role. This is a varied and rewarding community based role supporting clients to access services and also supporting group provision through our dual-diagnosis Calm Cafes, also on a referral only basis. You will play a key role in empowering individuals on their recovery journey, reducing isolation and improving their wellbeing.
Duties:
- Arrange and manage your appointments diary
- Provide home-based support tailored to individual recovery goals and referral requests
- Maintain accurate and updated case records on our database
- Support clients to access other services that are relevant to their recovery including providing transport and attending meetings with them
- Facilitate alongside other partner agencies the Dual-Diagnosis Calm Café for people to attend on a referral basis at different locations in Telford – this will be on the second Saturday of each month 1pm – 4pm
Experience / Qualifications:
- You must have a high level of mental health experience and a good knowledge of controlled substances to assess risks.
- You should be an efficient communicator and have good organisational skills to liaise with external agencies and internal stakeholders.
- You must hold a driving license and have own car with business insurance to support service users attend appointments and access services.
- You must be non-judgmental, empathetic, caring and committed to enable people to live their best life.
- Good IT skills for reporting including Teams, Sharepoint, Microsoft Office suite and any CRM database.
