Health and wellbeing coaches support patients to increase their ability to self-manage their health, increase their motivation levels and commitment to change their lifestyle. They are experts in behaviour change and focus on improving health related outcomes by working with people to set personalised goals and change their behaviours. They work with people with physical and/or mental health conditions and those at risk of developing them. They provide both one to one coaching and group sessions.

Health and Wellbeing Coaches offer effective interventions for people experiencing a range of long-term conditions, including respiratory, cardiovascular and stress/low mood. They can also support people with weight management, diet and increasing activity levels.

Health and Wellbeing coaches work in partnership with health, social care, community sector providers and multi-disciplinary team. This role plays an integral part in Primary Healthcare supporting patients to make positive choices working closely as part of the holistic care team with Social Prescribing Link Workers and Care Co-Coordinators.

The successful post holder will use a holistic approach to promote lifestyle approaches and support patients to address their existing health problems and prevent new ones. You will work closely with other members of the PCN holistic care team to identify individuals and groups who would benefit from your support.

Main duties of the job

Core Duties

Coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement and achieve their personalised health and wellbeing objectives. This will include sitting in consultation with a patient and providing them with advice, guidance, and a management plan that is personalised to their needs. It could include dietetics and healthy eating, lifestyle medicine and getting active, safe activities and personal coaching and motivation.

Manage and prioritise your own caseload in accordance with the health and wellbeing needs of the patient population.

Ensure all interventions and coaching are designed to empower patients to be active participants in their own healthcare; empowering them to manage their own health and wellbeing and live independently.

Support and deliver group consultations, in collaboration with clinical and non-clinical colleagues, implementing new approaches to health across primary care and supporting individuals to access peer support.

Work with the Social Prescribing Link Workers and the broader PCN colleagues to connect patients to community-based activities which support them to take increased control of their health and wellbeing.

Work with clinical colleagues to identify and provide enhanced support to those who would benefit the most from health coaching.

Provide a combination of in person, remote, telephone and video consultation.

Primary Care Networks (PCN) are local neighbourhoods where all health and social care services work together to deliver a better experience for their citizens.

Synergy Health PCN covers three practices in Gedling, Jubilee Park Medical Partnership in Carlton with a branch site in Lowdham, West Oak on Westdale Lane, Mapperley and Trentside Medical Group in Netherfield and Carlton.

We are a standalone PCN with a lead practice. We have a population of 36,000 patients.

The PCN has care co-ordinators, social prescribing link workers and pharmacy technicians to work alongside the health and wellbeing coach. These roles will work closely together as the PCN holistic care team.

Our vision is to build and develop collaborative working across member practices and stakeholders, to support and strengthen the delivery of Primary Care services and improve health outcomes for the local population we serve.
