Symphony Healthcare Services have a vacancy for a Practice Nurse, with experience in chronic disease management to work across Oaklands Surgery, Ryalls Park Medical Centre and Yeovil Health Centre in Yeovil, Somerset. Both are part of the Yeovil Primary Care Network of practices.

This is a permanent, part-time post of 30 hours per week. Days and hours of work will be discussed at interview stage. The rate of pay is £17.40 - £22.17 per hour (starting salary will be based on previous experience and specific qualifications), with the option to join the NHS or NEST pension scheme, plus other great employee benefits.
Main duties of the jobAs a Symphony Practice Nurse, you will provide and maintain a high standard of chronic disease management, which includes diagnosis, monitoring and development of individual management plans and agreeing these as appropriate with the patient and other health professionals.
- Be responsible for managing and running long-term condition clinics for your speciality.
- Be proactive in QoF and enhanced services management by completing annual reviews using specific templates and patient platforms.
- Review patients who present to the service on the day needing an acute review.
- Process pathology, workflow, tasks and assisting with audits as required.
- Refer directly to other services/agencies and members of multidisciplinary teams when needed e.g. social prescribers and health and well-being coaches.
- Supervise and provide training to other members of staff.

Job responsibilitiesYou will be an experienced Nurse, who, acting within their professional boundaries, will deliver high quality care for patients with a range of chronic diseases in the community setting. You will work with patients and their families to develop personalised goals, manage their condition themselves and lead as independent a life as possible.
You will work in partnership with the patient, their family and carers, to develop a personalised plan of care and will be responsible for monitoring and amending plans to meet the changing needs of the patient including referral to partner organisations such as palliative care, social care and social prescribing teams.
You will also work as part of a Multi-disciplinary team but will be responsible for a designated caseload of patients registered to the practice. They will demonstrate safe, clinical decision-making and expert care, and work collaboratively to meet the needs of patients and their family.
