Symphony Healthcare Services are looking for an enthusiastic and compassionate Nature-Based Health and Wellbeing Coach to join West Somerset Primary Care Network and lead the delivery of the Healthy Exmoor programme, a partnership with Exmoor National Park Authority.
You will be proactive, curious and collaborative, with the enthusiasm to develop partnerships and explore new ideas.
You will contribute to the continuing development of Healthy Exmoor as a place-based approach to personalised care and Green Social Prescribing, helping to create sustainable pathways between healthcare, nature and community.
This is a non-clinical role. You will receive regular supervision, ongoing training and opportunities for continuing professional development, and will be supported to work safely and effectively within the scope of the role.

This job role is a fixed term contract for 18 months, with the option to work between 30 and 37.5 hours per week., This is a unique opportunity to combine person-centered health and wellbeing coaching with the natural environment to support health.
You will provide personalised 1:1 coaching and facilitate group opportunities, supporting people to identify what matters to them, build confidence and self-management skills, and make positive and sustainable changes to their health and wellbeing.
Working across Exmoor National Park and the wider West Somerset community, you will help people to connect or reconnect with nature and their local communities in ways that support physical health, mental wellbeing, social connection, confidence and independence.
You will work creatively with existing opportunities and partners, while developing new nature-based approaches that respond to the needs and interests of local people.
You will work with a diverse range of people and communities, including those who may experience barriers to good health, social isolation or reduced confidence.
You will build trusting, compassionate and non-judgemental relationships, recognising the strengths and potential within each person and supporting them to become active participants in their own health and wellbeing.
