Symphony Healthcare Services are recruiting for an experienced and motivated Emergency Care Practitioner (Nurse or Paramedic) to join our friendly and forward-thinking Primary Care team at Crewkerne Health Centre in Crewkerne, Somerset.
Responsibilities
- Dedicated home visiting service (Mon-Fri): complete an average of 3-4 home visits per day for housebound patients and those unable to attend the practice, including assessment, management plans, safety-netting, documentation, and onward referral/escalation as needed.
- Urgent same-day face-to-face capacity (ad-hoc): run occasional on-the-day urgent appointments in practice (within agreed scope of practice/skill mix) to support GP capacity and patient access.
- Learning Disability (LD) reviews: undertake and/or support LD reviews in-practice across the year, helping to create protected capacity, improve completion, and provide consistent follow-up and care planning.
- Clinical coordination: triage and prioritise workload appropriately; liaise with GPs, nursing team, community services, and families/carers to ensure joined-up care.
- Safe prescribing and investigations (within competence): request/interpret relevant observations and investigations, contribute to treatment plans, and escalate clinical concerns appropriately.
- High-quality record keeping: ensure timely, accurate clinical documentation, coding, and communication of outcomes to the wider team.
- Support for vulnerable cohorts: provide proactive, patient-centred care for complex patients, including frailty, long-term conditions, and multi-morbidity, with clear escalation pathways.

Opportunity Details
This is an excellent opportunity for a highly skilled clinician to work autonomously within a supportive multidisciplinary environment, delivering high-quality urgent and same-day care to patients within a busy medical centre. This is a permanent, part time opportunity of between 25 and 30 hours per week. Competitive salary plus the option to join the NHS or NEST pension scheme and other great benefits, £55,450 to £64,046 a year - starting salary will be offered based on previous experience.
